Appointment Information

Please bring your insurance card and referral from your primary medical doctor (if needed) to your appointment. Don’t forget to also bring your most recent pair of glasses or contact lenses and list of current medications. If this is your first time at our office, or if we haven’t seen you for a long time, we may need to dilate your pupils. As this may cause some blurry vision and light sensitivity, you may wish to bring sunglasses and have someone accompany you to your exam.
